SIGNUP TERMS AND CONDITIONS FOR TELEMEDICINE APP

  1. ACCEPTANCE OF TERMS
    By signing up for an account on Sugamya Ayurveda (“App”), you (“User”) acknowledge and agree to be legally bound by these Terms and Conditions. If you do not accept these terms, you must not proceed with the registration.
  2. ELIGIBILITY
    2.1. You must be at least 18 years old to register. Minors may use the App only with the express consent of a parent or legal guardian.
    2.2. By signing up, you represent and warrant that you have the legal capacity to enter into this agreement.
  3. ACCOUNT REGISTRATION
    3.1. Users must provide accurate, complete, and up-to-date information during registration.
    3.2. The App reserves the right to verify User identity and may require additional documentation.
    3.3. Providing false or misleading information may result in account suspension or termination.
  4. ACCOUNT SECURITY
    4.1. Users are responsible for maintaining the confidentiality of their login credentials.
    4.2. Any unauthorised access or suspected security breaches must be reported immediately to the App’s support team.
    4.3. The App shall not be liable for any unauthorised access resulting from the User’s negligence.
  5. PRIVACY AND DATA COLLECTION
    5.1. By signing up, Users consent to the collection, processing, and storage of personal information as outlined in the Privacy Policy.
    5.2. The App may use User data for authentication, security, and service improvement purposes.
    5.3. The App employs industry-standard security measures but does not guarantee absolute protection against cyber threats.
  6. PATIENT REGISTRATION TERMS
    6.1. Users who register as patients must provide a complete and accurate medical history, including past and current health conditions, allergies, and medications.
    6.2. The App and its Providers rely on the accuracy of the information provided; any omission or misrepresentation may affect medical advice.
    6.3. Users acknowledge that virtual consultations may have limitations compared to in-person evaluations and accept the inherent risks.
    6.4. The App reserves the right to suspend or terminate patient accounts if any fraudulent activity or false medical information is detected.
  7. SELF-SIGNUP USERS TERMS
    7.1. Self-signup users acknowledge that they are solely responsible for the accuracy of their submitted personal and medical details.
    7.2. Users registering themselves for consultations understand that their data may be shared with healthcare providers within the App as per the Privacy Policy.
    7.3. Self-signup users cannot create multiple accounts for fraudulent activities, and any such attempts may lead to permanent suspension.
    7.4. By signing up, the User agrees that all consultations and recommendations provided via the App are for informational purposes and do not replace in-person medical evaluations.
  8. PATIENT APPOINTMENT BOOKING TERMS
    8.1. Patients may book appointments with healthcare providers available on the App subject to availability.
    8.2. The App does not guarantee the availability of any specific healthcare provider at a particular time.
    8.3. Patients must provide truthful and complete medical details before booking an appointment to ensure proper care.
    8.4. Appointment confirmations will be sent via email/SMS/app notifications, and patients are responsible for keeping track of their scheduled appointments.
    8.5. Cancellations and rescheduling must be made within the timeframe specified by the App. Failure to do so may result in penalties or restrictions on future bookings.
    8.6. No-shows without prior notice may result in account suspension or additional charges.
    8.7. The App is not responsible for any medical malpractice, misdiagnosis, or dissatisfaction with consultations; any disputes must be resolved directly between the patient and the healthcare provider.
    8.8. Payments for consultations, if applicable, must be made per the payment policies outlined by the App. Failure to complete payment may result in appointment cancellation.
    8.9. The App reserves the right to refuse service to any user violating these terms.
  9. TERMINATION AND SUSPENSION
    9.1. The App reserves the right to suspend or terminate User accounts for violations of these terms.
    9.2. Users may request account deletion, subject to applicable legal and regulatory obligations.
  10. MODIFICATIONS TO TERMS
    10.1. The App reserves the right to update these signup terms at any time. Users will be notified of significant changes via email or App notifications.
    10.2. Continued use of the App after modifications constitutes acceptance of the revised terms.

TERMS AND CONDITIONS FOR APPOINTMENT BOOKING

  1. APPOINTMENT BOOKING
    1.1. Registered users may book telemedicine appointments for themselves or on behalf of another person.
    1.2. Appointments are subject to the availability of healthcare providers, and the App does not guarantee the availability of any specific provider at a particular time.
    1.3. Patients must provide accurate and complete medical information before booking an appointment to ensure appropriate care.
  2. BOOKING ON BEHALF OF ANOTHER PERSON
    2.1. A registered user (“Booking User”) may book an appointment on behalf of another individual (“Patient”) only by uploading a valid government-issued identity proof and contact details of the Patient.
    2.2. The Booking User must ensure that the Patient has given consent for the appointment and the sharing of their medical information with the healthcare provider.
    2.3. The App reserves the right to verify the uploaded identity proof and contact details before confirming the appointment.
    2.4. Any misrepresentation or false information may result in appointment cancellation and suspension of the user’s account.
  3. CANCELLATION AND RESCHEDULING
    3.1. Cancellations and rescheduling must be made within the timeframe specified by the App.
    3.2. Failure to cancel or reschedule within the specified period may result in penalties or restrictions on future bookings.
  4. LIABILITY AND DISCLAIMER
    4.1. The App is not responsible for the accuracy of the information provided by the Booking User or Patient.
    4.2. The App is not liable for any disputes arising from the appointment booked on behalf of another person.
    4.3. The App does not guarantee the accuracy of medical advice provided by healthcare providers and shall not be liable for any medical malpractice or dissatisfaction with the consultation.
  5. COMPLIANCE AND ENFORCEMENT
    5.1. The App reserves the right to refuse appointment bookings if there is suspicion of fraudulent activity.
    5.2. Users who violate these terms may have their accounts suspended or permanently restricted.
  6. CANCELLATION AND REFUND POLICY

    6.1 Cancellation of the appointment is allowed before the time conveyed by Sugamya Ayurveda Pvt Ltd. But there will be no refund for the cancellation.

    6.2 Cancellation or modification of the cart is allowed till the medications are not shipped; after that, no modification or cancellation will be allowed.

    6.3 Refund for cancelled/modified cart will be as per the guidelines provided by Sugamya Ayurveda regularly, and Sugamya Ayurveda hold the right to change the policies, terms and conditions regularly to match the policies of shipping and transport companies.
